The Abstract form can be downloaded here. Please fill and send in the form with your abstract (please edit your abstract according to the requirement). After that, proceed to registration and reservation.

REGISTRATION FOR HERSHEY CONFERENCE (:

This reservation includes all meals, social events and receptions during the conference and resort fees. Payment can be made by going to the link provided below. Please read the detailed instruction before make any payment.

Only credit card payments will be accepted.

1. Registration for scientific participants: $550

2. Trainees $450* , use code "86C015" (same as guest) to get the discount rate.

*Trainees (students, residents, clinical fellows, graduate students and post-doc fellows) must provide a letter from their supervisor. They can email a PDF version to Rita Ghiselli at rghiselli@northshore.org. Registration may be voided if no letter is provided.

Snowbird mountain resort is offering the following options for guests who are accompanying the Conference participants (non-conference attendees).

Under the age of 5 free meals.

Over the age of 5 the options are:

All meals and social activities included is $326 per person.

There is an option for individual lunches and dinners for each day.

Dinner Buffet, Taste of Utah - $39.08 per person

Thursday

Lunch Buffet - $28.64 per person

Dinner Buffet, BBQ - $43.00 per person

Friday

Lunch Buffet - $28.64 per person

All payments have to be done by check (not credit card) and the check has to be received by May 28^th Friday. Please contact Rita Ghiselli for more detailed instructions on how to make the payment.

Cliff Lodge Bedrooms – Sleeps 4

Consist of two queen-size beds and a full bath. Rooms include a small refrigerator, hairdryer, iron/ironing board, humidifier, in-room safe and coffee maker. Bedrooms are approximately 360 square feet. $129.00 plus tax and resort fee.

Lodge at Snowbird Bedrooms – Sleeps 4

All rooms offer mountain views, balcony, two queen beds, one full bath and small refrigerator. Approximately 400 square feet. $89.00 plus tax and resort fee.

Lodge at Snowbird Studio Lofts – Sleeps 6

A studio-loft offers two queen beds upstairs and one queen sofa sleeper downstairs. Mountain view rooms with balcony, two full baths, gas fireplace, fully furnished kitchen. Approximately 835 square feet. $145.00 plus tax and resort fee.

ROOM CHARGES: Individuals Pay All Charges - Individuals will be responsible for payment of their own lodging, taxes and fees, additional nights and incidental charges at time of check-out. A credit card imprint will be taken at time of check-in.

LODGING DEPOSIT: A one-night deposit per room is required at the time the reservation is made. Full payment is required prior to departure.

LODGING CANCELLATION: Should cancellation be received within 48 hours of the scheduled arrival date, the entire one night’s lodging deposit will be forfeited. No-shows and early departures are considered a cancellation.

CHECK IN/OUT: Check-in time begins at 4:00 p.m. and is guaranteed by 6:00 p.m. Check-out time is 11:00 a.m. Luggage storage is available through bell service for guests arriving prior to check-in time and for those involved in meetings or activities that extend past check-out time.

AIRPORT TRANSPORTATION: Airport shuttles may be arranged through Snowbird Central Reservations (800-453-3000) up to 48 hours prior to arrival. Current round trip pre-paid price is $68.00 per person, plus driver gratuity. Prices are subject to change. Walk-ups are accepted on a space available basis at the Canyon Transportation desk located in the baggage claim area of the Salt Lake City International Airport. Cancellations received within 48 hours of arrival are non-refundable.
